What is Concussion & Why it needs to be taken Seriously?

All LFC coaches, managers, parents and players are strongly encouraged to view this special presentation by Vicki Evans who has been involved in Neuroscience nursing for over 30 years in a variety of positions - public and private, in Australia and New York, USA.

She is well known on the neuroscience lecture circuit – locally, nationally and internationally as well as for her many publications, including book chapters and journal articles.

Injury Procedure & Insurance

Every registered NSFA player is automatically covered by insurance through Football NSW’s Insurance Program, administered by Gow-Gates Insurance Brokers. This coverage is included in your registration fees and applies during official matches, training, trials, and other sanctioned football activities.

Injured? What do I have to do?  

  • At the time of injury, please send an email to the Registrar info@lindfieldfc.com.au informing Lindfield Football Club of your injury and copy your Age Coordinator. Our records need to be accurate so please ensure the incident is properly documented (date, time, where, when, game or training)

  • Visit the Gow Gates Claims Portal to download the Accident Claim Form - found under the menu item Lodgement of Claims. You will need to create an account for this process if this is the first claim you’ve made - otherwise sign in. Read the disclaimer.

  • Complete all relevant sections of the downloaded Club Declaration form - please do not send the club a blank document to fill in - this must be done by the claimant.

  • Send a PDF or Word Doc copy to the Club Registrar: info@lindfieldfc.com.au, who will verify and sign the Club Declaration and return it to you to continue the claims process.

  • Upload required documentation:

    • Club Declaration (signed by club representative)

    • Physician’s Report

    • Additional documents for loss of income claims

Important Notes

  • Medicare-related expenses cannot be claimed

  • Claims should be submitted as soon as possible after injury

  • Weekly benefits are payable for up to 52 weeks where applicable

Can I get a Refund?

  • If you are claiming on FNSW insurance, now, or in the future in relation to this injury, you must not Deregister. This will ensure that your insurance is paid and valid in relation to this injury.

  • If your injury is a season-ending one and you wish to claim a partial refund of the variable costs, please get in touch immediately so that we can organise.

  • Please email both your Age Coordinator (so they can remove you from the team) and the Club Registrar to advise of your situation.
